Contractors and Vendors
All new Contractors and Vendors are required to complete and return the forms listed and attach the documentation before you can be considered.
- Independent Contractor Information Form
- W-9 Request filled out
- Vendor Agreement Disclaimer of Liability Form
- Current Certificate of Insurance with Additional Insured & Sub Contractors Agreements must be submitted before work is started.
- Certificate of Insurance must have Workers Comp
